Everyone seems to be talking about testosterone. Podcasts, Instagram, longevity clinics, women's health influencers. Testosterone is being positioned as the answer for low libido, fatigue, brain fog, muscle loss, and motivation. Some of that excitement is warranted. Testosterone is a critical hormone in women, and its role in midlife health is worth understanding.
But testosterone rarely works the way the popular conversation suggests it should. Women start testosterone expecting energy, mental clarity, and drive. Some feel better. Many do not, or feel only partial improvement. The reason is not usually about testosterone alone. It is about the hormonal environment overall, and more specifically about what estrogen and progesterone are doing in the same woman during this timeframe.

What testosterone does in women, and where the evidence is strongest
Before discussing what testosterone can and can't fix, it's worth being precise about what it actually does.
Testosterone is produced in the ovaries and adrenal glands. It reaches peak concentrations in the 20s to early 30s and it declines gradually across a woman's adult life.¹ By the time of typical menopause (~age 51), women are at roughly 50–75% of their peak testosterone levels. The International Society for the Study of Women's Sexual Health (ISSWSH) and other guidelines specifically identify HSDD in postmenopausal women as the indication for testosterone therapy with the strongest supporting evidence.²
Muscle, energy, and mood. These are being actively studied but the evidence for testosterone as a treatment for these symptoms in women is preliminary. Some women report improvements. Randomized controlled trials have not consistently demonstrated benefit at the level of libido evidence. Bone density and cognitive function. Evidence is even more limited here. These are areas where researchers are exploring whether testosterone might have a role, but the evidence base does not yet support routine treatment for these indications.

Why estrogen and progesterone still come first for most women
Here is the clinical reality: for most perimenopausal and menopausal women, the biggest hormonal shifts driving symptoms are the changes in estrogen and progesterone, not testosterone.
Estrogen and progesterone shift dramatically during the perimenopausal transition. Estrogen becomes volatile, swinging from high to low in unpredictable ways. Progesterone often declines first, and its decline affects sleep, anxiety, and stress tolerance. These changes are instrumental to shaping the environment in which all other hormones are acting.
This is why, from a clinical perspective, addressing estrogen and progesterone often comes before adding testosterone to the landscape.
A woman whose estrogen is volatile and whose progesterone is insufficient may not get the response she is hoping for from testosterone, because the underlying hormonal environment may be more of the primary driver behind her symptoms. Once estrogen and progesterone are addressed through hormone therapy, testosterone as an addition can be more targeted and often more effective.

This does not mean testosterone is never appropriate on its own, and it does not mean every woman needs estrogen and progesterone therapy before considering testosterone. How the three hormones work together
Hormones do not act in isolation. They act as a network, and each one shapes how the others behave.
Estrogen creates a foundational environment. It supports brain function, insulin sensitivity, vaginal and urinary tissue, and how receptors throughout the body respond to other hormones, including testosterone. When estrogen is low or volatile, tissues are less able to respond to testosterone in the ways women are hoping for.
Progesterone stabilizes. It supports sleep, calm, stress tolerance, and recovery. What about testing testosterone levels?

What are your primary symptoms? Whether your concerns are sexual desire, energy, mood, focus, muscle, the estrogen and progesterone conversation is usually the more useful starting point. Testosterone can be added secondarily.
Are you already on hormone therapy for estrogen and progesterone? If so, then whether testosterone therapy adds meaningful benefit becomes a clearer question.
Who is prescribing? Testosterone therapy for women requires a provider who is familiar with the appropriate dosing, monitoring, and side effects. Because it is off-label in the US, the standard of care varies. Dosing that results in levels above the typical female range can produce androgenic side effects like acne, hair thinning, deepened voice or unwanted hair growth. What are your expectations? Testosterone can help with libido in the right situation.
